To ensure patient safety, legal compliance, and appropriate medical care, PharmXHealthOne requires all patients to meet specific age, identity, and medical eligibility criteria before receiving services.

 

By engaging with PharmXHealthOne, you acknowledge and agree to the following requirements:

 

1. Minimum Age Requirement

  • Patients must be 18 years of age or older to receive medical treatment, telehealth services, and prescription therapies through PharmXHealthOne.

  • PharmXHealthOne does not provide pediatric or minor care unless explicitly stated and authorized under separate clinical guidelines.

 

Patients under 18 will not be evaluated or treated.

 

2. Identity Verification

To comply with federal and state medical regulations, all patients must complete identity verification.

 

This may include:

  • Uploading a valid government-issued photo ID

  • Telehealth identity confirmation during consultation

  • Matching demographic information to submitted medical records

  • Providing documentation proving current state of residence

 

Failure to verify identity may result in refusal of services.

 

3. Residency & State-Specific Eligibility

Services and prescriptions are subject to state telehealth laws and pharmacy regulations. Therefore:

  • Patients must reside in a state where PharmXHealthOne is legally permitted to provide care

  • Some states restrict certain medications (e.g., compounded medications, controlled substances, peptides, or hormone therapies)

  • Certain states may require an initial in-person physical examination before telehealth prescribing

 

If your state prohibits a specific treatment, PharmXHealthOne may not be able to prescribe or continue that therapy.

 

4. Medical Eligibility Requirements

To qualify for treatment, patients must:

  • Provide a complete and accurate medical history

  • Disclose all medications, supplements, surgeries, and diagnoses

  • Complete any lab testing required for safety and clinical decision-making

  • Undergo a medical evaluation by a licensed PharmXHealthOne provider

  • Meet clinical criteria for weight-loss medications, peptides, hormones, or exosomal treatments

 

PharmXHealthOne providers may request additional labs, imaging, or documentation before prescribing.

 

5. Provider Discretion & Approval

Meeting age and basic eligibility criteria does not guarantee approval for treatment.

 

A PharmXHealthOne medical provider may:

  • Decline treatment

  • Modify the treatment plan

  • Require additional testing

  • Restrict medication options

  • Discontinue therapy

  • Refer the patient to in-person care

 

All treatment decisions are based on the provider’s professional judgment and applicable legal requirements.

6. Limitations for Certain Medical Conditions

Some conditions may limit eligibility for treatment, including:

  • History of medullary thyroid carcinoma (MTC) or MEN2

  • Active cancer or cancer history (depending on therapy)

  • Severe or unstable cardiovascular disease

  • Uncontrolled psychiatric conditions

  • Pregnancy or breastfeeding

  • Severe renal or hepatic impairment

  • Any condition deemed unsafe for telehealth management

 

The provider will determine suitability following clinical evaluation.

 

7. Patient Responsibility & Compliance

Patients must:

  • Provide accurate health information

  • Notify the provider of any new diagnoses, pregnancies, medication changes, or significant symptoms

  • Complete follow-up appointments and required laboratory testing

  • Follow all treatment instructions

  • Seek in-person or emergency care when appropriate

 

Failure to comply may result in suspension or termination of care.

 

8. Acceptance of Eligibility Requirements

By receiving care from PharmXHealthOne, you acknowledge and accept:
