By choosing to receive care through telehealth, online consultations, or telephone services, the patient understands that medical evaluation and treatment may be delivered remotely when appropriate and clinically acceptable. Remote care may involve limitations such as the inability to perform a physical examination and possible technology or communication issues. The patient acknowledges that they have been informed of the potential risks, benefits, and alternatives, and consent to proceed with remote care when deemed appropriate by their provider. The patient may withdraw consent at any time or request an in-person appointment when necessary. Remote care does not replace emergency services, and patients experiencing a medical emergency should call 911 or go to the nearest emergency facility.

Remote care may not replace all in-person visits. If an in-person evaluation is needed, we will schedule it.
