PHARMXHEALTHONE TELEHEALTH LIMITATIONS DISCLAIMER

PharmXHealthOne provides medical services through secure telehealth platforms in accordance with federal and state laws. While telehealth expands access to care, it has inherent limitations. By receiving telehealth services from PharmXHealthOne, you acknowledge and agree to the following:


1. Not a Substitute for In-Person Medical Care

Telehealth evaluations are limited to information shared electronically.
Telehealth does not replace:

  • Physical examinations

  • Emergency medical treatment

  • Urgent care visits

  • Hospital services

  • Comprehensive primary care


Patients should maintain ongoing care with a primary care provider for routine and preventative health needs.


2. Limited Diagnostic Capability

Telehealth providers rely solely on:

  • Patient-reported information

  • Visual assessment via video (if applicable)

  • Uploaded documents or images

  • Available laboratory data


Due to these limitations:

  • Certain conditions cannot be accurately diagnosed through telehealth

  • Providers may require additional labs, imaging, or an in-person evaluation

  • Treatment may be declined or deferred if safe care cannot be guaranteed remotely


3. Medication Limitations

Telehealth prescribing laws vary by state. Therefore:

  • Some medications cannot be prescribed via telehealth in specific jurisdictions

  • Some states require an initial in-person physical exam for certain prescriptions

  • Controlled substances are regulated and may require additional steps or may not be available at all in certain states

  • Compounded medications may be restricted or prohibited depending on local pharmacy regulations


PharmXHealthOne complies strictly with all state-to-state prescribing rules.


4. Technology & Communication Limitations

Telehealth may involve risks including:

  • Poor internet connectivity

  • Audio or visual distortions

  • Technical interruptions

  • Delayed messages or information transmission

  • Unauthorized access despite security safeguards


PharmXHealthOne uses secure, HIPAA-compliant systems but cannot guarantee absolute cybersecurity.


5. Privacy Limitations

Telehealth requires the use of electronic communication.
Though every effort is made to protect your privacy:

  • Your device, internet provider, or environment may introduce privacy risks

  • Patients are responsible for ensuring they conduct telehealth visits in a private, secure setting


6. Emergency Situations

Telehealth is not appropriate for emergencies.
If you experience:

  • Chest pain

  • Shortness of breath

  • Severe pain

  • Loss of consciousness

  • Uncontrolled bleeding

  • Suicidal thoughts

  • Any life-threatening condition


You must call 911 or go to the nearest emergency room immediately.

PharmXHealthOne does not provide emergency medical services via telehealth.


7. Local Service & Physical Procedure Limitations

Telehealth cannot be used for:

  • Physical examinations requiring hands-on assessment

  • Equipment-based procedures

  • IV therapy outside Florida

  • Body contouring or laser treatments

  • Any treatment requiring direct provider supervision or specialized equipment

Procedures and IV therapies are available only at the Boynton Beach clinic or within a 20-mile mobile radius.


8. Provider Discretion

PharmXHealthOne providers reserve the right to:

  • Decline treatment if telehealth is not sufficient

  • Require additional medical records or labs

  • Refer the patient to in-person care

  • Discontinue telehealth services if necessary for patient safety

Completion of a telehealth consultation does not guarantee prescription approval or continuation.


9. Patient Responsibility

Patients must:

  • Provide complete and accurate medical information

  • Upload clear and accurate photos or documentation when requested

  • Notify their provider of any health changes

  • Complete labs and follow-up tasks as required

  • Seek urgent or in-person care when advised

Failure to follow these responsibilities may limit the effectiveness and safety of telehealth care.


10. Acceptance of Telehealth Limitations

By using PharmXHealthOne’s telehealth services, you acknowledge that:

  • Telehealth has inherent limitations compared to in-person care

  • Certain diagnoses and treatments cannot be performed remotely

  • Treatment decisions are based solely on information available at the time of evaluation

  • You understand the risks, benefits, and limitations of telehealth


You agree to seek in-person medical attention whenever necessary for safe and appropriate care.

Telehealth Consent

What is Telehealth?

Telehealth allows you to consult with licensed healthcare providers—including physicians, nurse practitioners, physician assistants, and mental health professionals—remotely using secure audio and video technology via your phone, computer, or tablet. This enables care from the comfort and convenience of your chosen location.

 

Benefits of Telehealth

  • Reduces physical contact and lowers exposure to infectious diseases.

  • Receive care from anywhere—home, work, or on the go.

  • Eliminates the need for travel, time off work, or childcare.

  • Shorter wait times for appointments.

  • Access to specialists who may not be available locally.

 

Potential Risks of Telehealth

As with any healthcare service, telehealth may involve certain risks:

  • Technical limitations (e.g., poor video quality) may hinder diagnosis or treatment.

  • Equipment failures may delay care or result in incomplete assessments.

  • In rare cases, privacy breaches may occur due to internet security vulnerabilities.

  • Limited physical exams may affect the accuracy of the provider’s assessment.

  • Your provider may determine that an in-person visit is necessary.

  • The video or audio connection may fail during your visit.

  • Patients are responsible for ensuring their internet connection and device are secure and private.

 

Privacy and Confidentiality

  • Telehealth visits are not recorded.

  • Your provider will document the session in our secure electronic medical record system.

  • It is your responsibility to be in a private, quiet space where others cannot overhear your session.

  • Your provider will inform you if anyone else is present on their end.

  • While PharmXHealthOne uses HIPAA-compliant technology, no internet-based system can guarantee 100% privacy.

 

Your Rights

  • You may decline or discontinue telehealth at any time, including during a visit.

  • Opting out of telehealth will not affect your access to future care.

  • Telehealth is not appropriate for emergency medical or psychiatric care. In case of emergency, call 911 immediately.

 

Telehealth Visit Cost

PharmXHealthOne offers free initial telehealth consultations and no-cost follow-ups when you purchase a treatment through our platform.

 

Consent Acknowledgment

By participating in a telehealth consultation with PharmXHealthOne, you acknowledge and consent to the terms above and understand the benefits and limitations of receiving care through telehealth.
