PharmXHealthOne provides consultation services through licensed medical professionals. By scheduling an appointment, you agree to the following terms: 1. Consultation Services All scheduled appointments are consultations only and are intended for: Medical evaluation Discussion of health concerns Determination of eligibility for potential treatment options No diagnosis, treatment, or prescriptions are guaranteed and are provided only when deemed medically appropriate by a licensed provider. 2. No Upfront Charges At this time, no fees are collected prior to consultation services being rendered. If treatment, medications, or additional services are recommended, all costs will be clearly discussed and require your consent before proceeding. 3. Provider Review & Eligibility All care decisions are made solely by licensed medical providers based on: Medical history Current health status Applicable clinical guidelines State-specific regulations Scheduling an appointment does not guarantee eligibility for any specific treatment or service. 4. Appointment Responsibility By booking an appointment, you agree to: Provide accurate and complete information Be available at the scheduled time Ensure access to phone or internet (for virtual visits) Failure to attend may limit future booking availability. 5. Late & Missed Appointments If you are more than 10 minutes late, your appointment may need to be rescheduled Repeated missed appointments may result in restricted booking access 6. Cancellations & Rescheduling We request at least 24 hours notice for cancellations or rescheduling. This allows us to make appointments available to other patients in need of care. 7. Non-Emergency Services PharmXHealthOne does not provide emergency medical services. If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room. 8. Communication Consent By booking an appointment, you consent to receive: Appointment confirmations Reminders Follow-up communications related to your care 9. Privacy & Confidentiality All patient information is handled in accordance with HIPAA and applicable privacy laws.
