This registration form gives PharmXHealthOne Integrated Medical Clinic (PHO) the information needed to identify you, contact you, schedule care, document your medical history, coordinate with pharmacies and laboratories, and determine whether PHO can safely evaluate or treat you. Please come in at least 15 minutes before your scheduled appointment to complete check-in process before your consultation appointment.

Consultation & Treatment Package Payment Policy

All patients who book a consultation appointment are required to pay the consultation fee before the consultation. If a PHO medical provider prescribes a treatment package and the patient chooses to move forward, the full treatment package amount is due at the time of purchase. Payment installment available through third-party, Affirm and Afterpay.
